Important Tuition Assistance Application Information
To apply for the Pennsylvania Children's Tuition Fund all required documentation MUST be submitted to Jubilee Christian School's office prior to deadline.
All Applicants must submit:
- Completed Family Application Form (see Tuition Assistance Application tab above)
- Completed Financial Information Form (see Tuition Assistance Application tab above)
- Most recent copy of you 1040 Tax Return (if you were required to file income tax)
or- A copy of your Public Assistance Statement or other proof of income along with your Financial Information Form

*note: A W-2 is NOT acceptable.
Failure to return all required documentation to Jubilee Christian School prior to deadline will result in failure to qualify for ACSI Tuition Assistance.
