Required Documentation for Enrollment
Jubilee Christian School enrollment is limited. Enroll early to ensure your child's place.
In order to enroll your child, you must present the following:
- Birth Certificate
- Immunization records - Kindergarten students entering school for the first time are required to show proof of the following
immunizations:
- Diphtheria Tetanus - 4 doses required, 1 dose to be given on or after the 4th birthday.
- Polio - 3 doses required (OPV).
- Measles/Mumps/Rubella - 1 dose to be given on or after the 1st birthday; 2 doses of measles vaccine required, usually given as
MMR.
- Hepatitis B - 3 doses required, properly spaced.
- Proof of Varicella immunity (Chicken Pox).
- All required forms of Jubilee Christian School
- Transfer students are admitted based on a conference, screening, and current class enrollment.
